Preventing Hospitalizations And Relapse
Community Solutions believe it is healthier for persons with TBI, MI, CD and related conditions to live in the community rather than in an institutional setting, such as a hospital. Preventing hospitalization is one of the primary goals of our program. We see medication compliance and side effect monitoring as a key factor in preventing hospitalization.
Medication supervision and education is essential to this program. Staff is trained in medication supervision and the medications commonly prescribed to treat mental illness. Trained staff members teach and assist clients to maintain a healthy lifestyle. We offer an atmosphere where clients can share their concerns about their disabilities, dependency and conditions, medications, progress, etc...
We offer information and experience in addressing such issues as how to talk to doctors and counselors, diagnoses, medications, warning signs of de-compensation, community resources, problem solving, decision making, community living skills, physical health and wellness, symptom management, coping skills, and relapse prevention skills. Residents have the opportunity to attend AA and or NA as often as they like. We also assist clients in connecting to medical and psychological professionals.
